If you are the parent or guardian acting on behalf of a minor under the age of 13, please use this form.
If the consumer is a minor at least age 13 but less than age 16, please use this form.
If you are the Authorized Agent acting on behalf of a consumer at least 13 years of age, please use this form. Authorized Agents must be at least 18 years of age.
If you have a valid Power of Attorney for the consumer for whom you are making a CCPA request, please submit a copy of the Power of Attorney document along with the CCPA form (accessible via the “exercise your rights” link above), by mail, or by taking it to your local Patelco branch.
